Hall of Fame Nomination Form

Please nominate your favorite former MHS athlete by using this FORM – Please share any contact information for the athlete, his/her years at MHS, awards, sports played etc. that you are able to complete this FORM. The induction ceremony is typically held in the fall each year, so any nominations received after June 30 each year will be considered for the following year.

Coaches must no longer be employed with the schools, are retired, or have left this school system.

Additional submissions do not increase the chances of the nominee being voted in by the committee.
